I am currently reviewing this with the other Engineers but this is what I have for you towards your questions so far:

(1). Such a document was not published but I am working on getting a (soft copy) made.

(2). Being reviewed.

(3). Insertion being reviewed - however you can press the SAVE/ENTER and EXIT buttons together at the end of each midifile that you play. This will remove any insertions and delete any hanging notes.
