There are many programs that can add text to a midifile. Not all of them follow the specifications as given by the MIDI Manufacturers Association, see: http://www.midi.org/about-midi/smf/rp017.shtml
So I guess you have some files that aren't fully compatible with the SMF+lyrics standard.
