"Init phase error" - Have them check out HD and cable from HD to CPU board thoroughly!

Forward the following information to FC Musica (if they don't have it already) for Sound board issue(s).

* Press DISK
* Goto System and press ENTER (twice)
* Go to Factory and press ENTER
* Go to Sound and press ENTER.
* Select the first *.pcm file and press enter to load.
* When done, come back and select the first *snd file and press load.

This should reset your Sound card, otherwise they should simply have it replaced. You seem to be having issues with your Flash memory and I am not sure what enviromental factor is causing this.
